Description: The risk management standards for quality documentation are the same whether practicing in paper, electronic or a hybrid record environment. In an electronic environment, a good risk management plan goes beyond creating or entering information into the EHR. Planning and maintaining the integrity of the medical record is critical in patient safety efforts and in defending medical malpractice claims.
